New enterprise server setup issue - cannot generate intermediate CSR

Hello,  we are testing the 2factor server in preparation for enterprise deployment.  When I click on create an intermediate CA, the webpage does not display a form.  We used the ISO to setup the server on VMware 5.5.  Any ideas on how to troubleshoot?

And you're using the most recent 4.1 version?

Most likely this is a problem with the hostname. Please rerun 'wikidctl setup' and be sure that the hostname is correct.

Yes, this is the most current version.  What should the hostname be.  This environment is not public and does not have a domain.  The hostname I used was Wikid.  But I am accessing it through IP.

Check the tomcat logs by running 'more /opt/WiKID/tomcat/logs/catalina.err' and see if there's an error there.
